(1601 East Hazelton Avenue) • Stockton,California 95201 <br /> c� i 6' `P <br /> (209) 468-3400 <br /> TO: INSTALLERS AND OWNERS OF NEW UNDERGROUND STORAGE TANKS <br /> San Joaquin County Public Health Services-Environmental Health Division(PHS-EHD)has <br /> established the following guidelines to assist you in the completion of your construction <br /> project. The guidelines specify the construction and monitoring criteria necessary to <br /> complete our review of your proposed underground storage tank (UST) system. <br /> 1. SUBMITTAL OF PLANS <br /> a. Submit 4 complete sets of detailed drawings in non-erasable ink to the <br /> Environmental Health Division. Upon approval, one set will be retained by <br /> the department and the other three will be returned. <br /> b. Include equipment and material specification sheets and manufacturer's <br /> brochures describing operating and monitoring systems and listing numbers or <br /> other evidence of a nationally recognized testing organization approval. <br /> C. Include detailed monitoring and response plans for facility. <br /> d. The drawn to scale plot plan should illustrate the location of all existing and <br /> proposed structures, including existing UST(s). <br /> e. The plans should include a cross sectional diagram of the tank and piping <br /> system including secondary containment, overflow protection equipment, <br /> extension of all pipes, and ports from subgrade to finish grade. <br /> 2. PLAN REVIEW AND OPERATING PERMIT FEES <br /> a. New facility plan review fee of$30, operating permit application fee of $100, <br /> $50 fee per tank, plus $56 state surcharge fee per tank is due at time of plan <br /> submittal. <br /> b. New installation of additional tank at existing facility is $30 plan review fee <br /> plus $50 fee per new tank, and $56 per tank state surcharge fee. <br /> C. The maximum review time for UST installation plans is 40 working days. A <br /> preliminary review will be conducted by the PHS/EHD within 20 working <br /> days of submittal date to identify any gross plan deficiencies. <br /> d. If gross deficiencies are identified, the plans will be returned with checklist <br /> identifying plan deficiencies. <br /> e. It should be noted that a $30 plan review check fee required for each <br /> submittal allows for up to two hours of staff review time. Additional time <br /> spent on plan review, meetings,inspections,phone or office consultations will <br /> be billed to the applicant at a rate of $35 per hour. <br /> f. To expedite the review of installation plans, a $52.50 per hour overtime fee <br /> may be submitted to have review completed within 10 working days of <br /> submittal date. <br /> A Division of San Joaquin County Health Care Services <br />
