I have received your <br /> fax dated August 18, 1999 with the Line Leak Detector information. Please submit the <br /> following in order to continue to permit process: <br /> 1) You have indicated that you will be installing your own tanks under you're A license. <br /> PHS/EM requires that all contractors installing UST's have a hazardous <br /> endorsement on their license. PHS/EHD requires certificates of training by the <br /> piping manufacturer and the UST manufacturer prior to issuance of permits to <br /> contractors. Please submit proof of a hazardous endorsement and copies of training <br /> certificates. <br /> 2) The prints,the "B" form, and the permit application indicate that the UST to be <br /> installed is a 64 foot and 3/ inches long, 10 foot 4 inch diameter, 35,000 gallon <br /> Xerxes model number MTO.The submitted manufacturer specification sheet for <br /> this tank indicates it to be a 55 foot 9 3/ long, 10 foot 4 inch in diameter, 30,000 <br /> gallon Xerxes tank model number TD-20. Please specify the tank you will be using <br /> and amend plans and application and form "B", or submit the proper specification <br /> sheet. <br /> 3) Please submit installation instructions for the tanks,piping and monitoring devices. <br /> 4) The dispenser sensor indicated in the plans is not on the LG 113-13 Est and <br /> therefore is not approved for this system.Please submit manufacturers specifications <br /> for a dispenser pan sensor which is on the list.An alternative solution would be to <br /> use shallow dispenser pans that do not require sensors if they have a float shut off at <br /> the sheer valve. <br /> 5) Manufacturers specifications for spill containers has not been provided. Spill bucket <br /> must have a minimum capacity of 5 gallons. <br /> 6) Please provide the manufacturers specifications for the rubber flex boot. <br /> �/� �r� <br />
