The following is an itemized list of underground storage tank violations that have not been addressed for GAS DEPOT #2 as of May 06, 2017. Open violations from December 29, 2016 inspection Violation#201 -Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The site is not maintaining alarm logs or maintenance logs for alarms addressed by the site without a technician. The alarms include: L1 (UDC 1/2) 1/5/16 Setup Data Warning L11 (91 fill) 1/18/16 Fuel Alarm L11 (91 fill) 3/22/16 Fuel Alarm L12 (87 STP) 1/5/16 Liquid Warning According to Mr. Hur, he tried responding to the 1/5/16 alarm, but did not know how to determine which sensor was having a problem or how to silence the alarm. He stated that in trying to silence the alarm on the monitoring panel, he caused a set up data warning on the UDC 1/2 sensor. Work orders were found on site for a technician resetting the Setup Data Warning for UDC 1/2. Mr. Hur stated that for all three liquid alarms, he pumped the liquid from the sump into a waste drum kept behind the store. No records were available for these responses. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. Violation#204-Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. All copies of the monitoring plan on site were many years old and were outdated. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. Violation#205 -Failure to have an approved UST response plan available onsite. An approved copy of the response plan was not available for inspection. A copy of this plan shall be accessible on site at all times. Immediately locate and retain a copy on site. Violation #314-Failed to have secondary containment or secondary containment not tight. According to a phone call from Kim with Elite IV on January 30, 2015, three UDCs failed secondary containment testing on January 29, 2015 and permits were required for repairs. No permits were obtained for UDC repairs and no work orders were found on site for UDC repairs. All secondary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. Open violations from December 30, 2015 inspection Violation#110-Failed to submit secondary containment test report within 30 days. Secondary containment testing was scheduled to be performed on 1-29-1015 and a test report has not been submitted to the EHD. A copy of the test report must be submitted within 30 days of the test. Immediately submit a copy of the test report to the EHD or schedule this test and provide 48 hours notification to the EHD. Violation#115-Failed to inform the EHD of a change of designated operator within 30 days. According to Mr. Hur, a new designated operator was hired sometime in November 2015 and a notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. Violation#204-Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection.
