This information was required to be submitted by February 4, 2016. <br />This information has not been received resulting in a non-compliant status for this facility. The outstanding violations <br />include: <br />-Monitoring and response plans not current or approved by the EHD. <br />-Failed to submit secondary containment test report within 30 days. <br />-Failed to inform the EHD of a change in designated operator within 30 days. <br />-Current monitoring plan approved by the EHD not found on site. <br />An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br />statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br />written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br />inspection report. <br />This is a repeat violation, Class Il. <br />201 <br />CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. The site is not maintaining alarm <br />logs or maintenance logs for alarms addressed by the site without a technician. The alarms include: <br />L1 (UDC 1/2) 1/5/16 Setup Data Warning <br />L11 (91 fill) 1/18/16 Fuel Alarm <br />L11 (91 fill) 3/22/16 Fuel Alarm <br />L12 (87 STP) 1/5/16 Liquid Warning <br />According to Mr. Hur, he tried responding to the 1/5/16 alarm, but did not know how to determine which sensor was <br />having a problem or how to silence the alarm. He stated that in trying to silence the alarm on the monitoring panel, he <br />caused a set up data warning on the UDC 1/2 sensor. Work orders were found on site for a technician resetting the <br />Setup Data Warning for UDC 1/2. Mr. Hur stated that for all three liquid alarms, he pumped the liquid from the sump <br />into a waste drum kept behind the store. No records were available for these responses. <br />These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of <br />all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br />observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br />site and submit copies to the EHD. <br />This is a repeat violation, Class II. <br />204 <br />CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. All copies of the monitoring plan on site <br />were many years old and were outdated. A copy of these plans shall be accessible on site at all times. Immediately <br />locate and retain a copy on site. <br />This is a repeat violation, Class II. <br />Page 5of7 <br />
