A copy of this plan shall be accessible on <br />site at all times. Immediately locate and retain a copy on site. <br />This is a repeat violation, Class ll. <br />210 <br />CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The April 2016 and October 2016 designated operator monthly inspection reports were not found on site. The <br />designated operator had them emailed over at the time of inspection; Mr. Hur was unable to print them for his on site <br />records, so review of the missing inspection reports was conducted using copies on the designated operator's phone. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Ensure that <br />copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. <br />This is a repeat violation, Class ll. <br />314 <br />HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br />According to a phone call from Kim with Elite IV on January 30, 2015, three UDCs failed secondary containment <br />testing on January 29, 2015 and permits were required for repairs. No permits were obtained for UDC repairs and <br />no work orders were found on site for UDC repairs. All secondary containment for the UST system must be tight. <br />Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under <br />permit and inspection of the EHD. <br />This is a Class II violation. <br />315 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the 91 sump. If water could enter into the secondary containment by precipitation or <br />infiltration, it must be removed and disposed of properly. The service technician removed several inches of liquid <br />from the 91 fill sump. Ensure that all sumps and annular spaces are maintained free of liquid. <br />This is a Class II violation. <br />318 <br />CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br />a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />Page 6 of 7 <br />
