Complete maintenance and monitoring records for the last three years were not found on site. No alarm logs were available for the L9 and L10 alarms which occurred on January 29, 2015. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Ensure that all maintenance and monitoring records are kept for the last three years. This is a Class II violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a Class II violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document all the alarms from the attached alarm history on the February 20, 2014 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L10 alarm on January 17, 2014. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This was corrected on site, refer to violation 125. This is a repeat violation, Class II. 208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. The January 2015 to November 2015 designated operator monthly inspection reports were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Ensure that designated operator inspections are being conducted monthly, locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. This is a Class II violation. 319 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). Liquid was observed in the 91 fill sump, the 91 STP sump and the 87 fill sump. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed approximately 2 cups of liquid for the 91 fill sump, approximately 5 gallons of liquid from 91 STP sump, and approximately 0.5 gallons from the 87 fill sump. Ensure that all sumps and annular spaces are maintained free of liquid. This is a repeat violation, Class II.
