Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />Your permit application to install a new spill buckets at the above referenced location has been <br />approved with the following conditions: <br />1. The Supreme Spill Bucket must be replaced immediately. A re -inspection will occur <br />within 7 days of today. <br />2. Contact EHD to schedule the following appointments: <br />a. Contact EHD prior to backfilling the new spill buckets, EHD must inspect the <br />riser for cathodic protection. <br />b. The spill buckets must be tested to ensure they hold a minimum of 5 -gallons. <br />3. A tank integrity test must be performed once concrete is poured, prior to the contractor <br />signing off of the jobsite. This office must receive results within 7 days of completing the <br />work. <br />4.
