A <br />999 Other: See attached "Continuation — Official Inspection Report" for violation(s) not listed above. <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />This revised inspection report supersedes the initial inspection report issued on 6-20-14. <br />303. The 87 stp sump sensor was touching the bottom of the sump, approximately 5 inches away from <br />the lowest point of the sump, located part way up the saddle of the tank sump, and not located to detect <br />a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak <br />at the earliest possible opportunity. During the inspection the technician repositioned the sensor to the <br />lowest point of the sump so that it could detect a leak at the earliest opportunity. <br />305. The 87 Bravo box float and chain in the UDC #1/2 sump failed to stop the flow of product at the <br />dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br />alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted <br />the 87 chain and verified functionality. <br />306. A fuel leak was found at the diesel fuel filter in dispenser #1/2. All primary containment must be <br />product tight. During the inspection the technician tightened the diesel fuel filter to stop the leak. During <br />the inspection, the technician removed the spilled diesel from the UDC #1/2 and placed it into the <br />hazardous waste drum on site. <br />306. Approximately one pint of diesel was found in the diesel stp sump. All primary containment must be <br />product tight. Immediately have a properly licensed, trained, and certified contractor trouble shoot the <br />cause of diesel entering the diesel stp sump. Depending on the scope of work, a permit and inspection <br />from the EHD may be required.
