S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Staples the Office Superstore#1341 249 COMMERCE AVE, MANTECA June 03, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented)that <br /> employees who handle hazardous waste were properly trained based on the following observations: <br /> 1.A bagged bottle of spilled paint was observed in a container labeled as hazardous waste oxidizer and with an <br /> accumulation start date of 11/4/2019.The label was missing the physical state and composition (contents). Rick <br /> Cormier,the store manager, marked the physical state and contents at the time of inspection. <br /> 2. Two bags of alkaline batteries were observed in a container labeled as hazardous waste corrosive base.The <br /> label was missing the accumulation start date and the composition (contents). Rick stated that the alkaline <br /> batteries were left at the store by a customer around the time Rick came back to work from his leave of absence. <br /> Rick removed the batteries from the hazardous waste corrosive container and placed them into a container labeled <br /> as universal waste and with the generator name and address. Rick labeled the universal waste container with the <br /> contents and an accumulation start date of 3/2/2020 at the time of inspection. <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with <br /> proper waste handling and emergency procedures, relevant to their responsibilities during normal facility operations <br /> and emergencies. <br /> CORRECTIVE ACTION: Immediately train or re-train the employees. Provide proof of training or re-training, and <br /> training materials,within 30 days to the San Joaquin County Environmental Health Department(EHD)for <br /> employees whose responsibilities include hazardous waste. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> -According to facility manifests observed at the time of inspection, the amount of hazardous waste generated in <br /> 2019 was .001 tons. <br /> -OBSERVATION: A complete modified contingency plan with an assigned emergency coordinator was not <br /> posted at the facility. <br /> REGULATION GUIDANCE: The emergency coordinator or his designee must respond to any emergencies that <br /> arise. The business operator must post the following information next to telephones or in areas directly involved <br /> in the generation and accumulation of hazardous waste: the name and emergency telephone number of the <br /> emergency coordinator, the location of fire extinguishers and spill control material, and the telephone number of <br /> the fire department unless the facility has a direct alarm. <br /> CORRECTIVE ACTION: Rick Cormier, store manager, posted the required information near the customer <br /> service area at the time of inspection.
