Advanced GeoEnvironmental, Inc.
November 17, 1999
On November 10, 1999,PHS/EHD was additionally informed that MW-7,the
northernmost MW in the work plan would not be completed until the analytical
results of the soil samples from the other monitoring wells were reviewed and it was
determined that MW-7 was still necessary. PHS/EHD was not consulted before these
decisions were made.
At the meeting on November 16, 1999, PHS/EHD was informed that AGE planned to
install another MW not proposed in the work plan next to MW-6, which was installed
in the continuous cored bore-hole. As part of the work plan approval,PHS/EHD
provide the condition that the proposed boring (continuous core boring) near B5
should advance to at least 30,feet below surface grade with the contingency to bore
deeper if necessary to provide vertkA.definition and that if>it was necessary a MW be
installed in separate boring. Installation of a second separate.MW next,to MW-6
deviates from this condition.
Communication about site corrective action decisions and the scheduling of work
between PHS/EHI) and AGE must improve. PHS/EHD approval should be obtained
.before any modifications to the work plan are made. Notification to,PHS/EHD of
scheduled work should be,Made, when AGE has every indication:that the work will.
,commence on the.propose&date. .
If you have any questions please co Jeffrey Wong av'(209
contact T
.. 468-03,35.
Donna Heran, REHS,.Director:
Environmental Health Division A
4
X.
Jeffrey Wong,REHS Margaret> agorlo, REHS
on
. :Unit TV Supervisor
CC PHS/El-ID-:Lori Duncan
