Well MW-3A was installed as a replacement for well MW-3 which was
abandoned on November 24, 1992 in preparation for the tank pull and soil excavation
program that occurred at the site. Well MW-3A was drilled and installed on November
29, 1993 in accordance with the work plan by Mittelhauser entitled Monitoring Well
MW-3A Installation Work Plan, dated November 10, 1993.
Well MW-3A was installed by Bayland Drilling (supervised by a Mittelhauser geologist)
under San Joaquin County Public Health Services permit number 001574 (attached).
Mr. Harlin Knoll of San Joaquin County observed the installation of the grout seal and
well box.
. The casing consisted 15 feet of 0.02-inch slotted PVC under 5 feet of
blank PVC. A traffic rated well box was installed over the well at the surface. The
Boring and Well Construction Logs are attached to this report.
The subsurface soil was sampled every 5 feet during drilling and logged by a qualified
geologist according to United Soil Classification System standards. The soils
encountered consisted of clayey silt above a depth of 14 feet and sandy silt below a
( depth of 14 feet. Soil samples from each 5 foot interval were screened for volatile
Your Partner in Environmental Management
LM Chicago, IL ■ Indianapolis, IN 0 Laguna Hills, CA ■ Pleasanton, CA
