Elena Manzo , REHS <br /> Page 2 <br /> Issue # 118 CCR , Section 66265 . 16(a -c) — Facility failed to main, <br /> complete personal training records. � <br /> DVI is conducting safety training with all employees on a regular monthly, <br /> quarterly and annual basis . Individual training is also provided on an as needed <br /> basis. The appropriate paperwork is completed for each employee attending <br /> training and available in the In-Service Training database , work areas , <br /> supervisory employee' s files , and the Fire Department. <br /> However, in accordance with the requirements of the California Code of <br /> Regulations , Title 22 , Section 66265 . 16 (d ) , DVI developed a summary training <br /> spreadsheet with : <br /> • employees' names <br /> • job title <br /> • job description <br /> • training required and , <br /> • training provided for each employee involved in hazardous waste <br /> operations . <br /> The spreadsheet has been uploaded to the CERS website for review and to <br /> advise DVI if anything else needs to be done to improve our recordkeeping <br /> practices . <br /> Issue # 301 CCR, Section 66265 .31 and #303 CCR, Section 66265. 33 — <br /> Facility failed to minimize the release of a hazardous waste. <br /> 1 . The oily stain was cleaned . The building is no longer in use and has been <br /> secured to prevent access . <br /> 2 . The soil contaminated with oil has been collected in drums and removed . <br /> 3 . The lids on the oil containing drums have been cleaned . <br /> 4 . The oily stain on the 2000 gallon diesel storage tank has been cleaned <br /> 5 . The white deposit has been cleaned off the pipes located in the chemical <br /> feed pump area . <br /> 6 . The oily staining noted on the pavement has been cleaned . <br /> 7 . The pooling of an oily liquid on the blue 5 gallon container has been <br /> cleaned and removed . <br /> All of the waste accumulated as a result of the cleaning the above has been <br /> removed (shipped out to Industrial Waste Utilization) . Shipping generator <br /> manifests are attached . Training has been provided to PIA and Waste Water <br /> Treatment Plant operators regarding housekeeping practices and treatment of <br /> waste at their work sites . <br /> Issue # 303 CCR, Section 66265.35— Facility failed to maintain adequate <br /> aisle space for unobstructed response to emergencies. <br /> 1 . The pooling of oily liquid in the secondary containment pans was cleaned <br /> and removed . <br /> 2 . The oily staining on the ground and the oily liquid inside the secondary <br /> containment pans near the farm equipment maintenance shop were <br /> cleaned . <br /> 3 . The standing liquid inside the secondary containment in the dairy station <br /> area was cleaned . <br /> All of the waste accumulated as a result of the cleaning the above has been <br /> removed (shipped out to Industrial Waste Utilization) .
