Pre-field Activities <br /> • A pre-work site meeting was conducted on August 12, 2013, during which Terry Fox with <br /> Caltrans and Josh Ewert with Geocon reviewed the project site and scope. The proposed UST <br /> excavation area was marked out with white paint for subsequent utility clearance by responding <br /> Underground Service Alert(USA)members. <br /> • Contacted USA (Ticket No. 449615) a minimum of 48 hours prior to performing the field <br /> activities. <br /> • Obtained a soil boring permit from the SJCEHD.A copy of the permit is attached. <br /> • Prepared a Health and Safety Plan dated October 2013 to provide guidelines on the use of personal <br /> protective equipment and the health and safety procedures implemented during the field activities. <br /> • Prepared a Workplan dated October 8, 2013, to describe the requested scope of services and to <br /> outline procedures and methods to complete the field sampling and analytical testing activities. <br /> • Retained the services of Advanced Geological Services (AGS) to perform a geophysical and <br /> utility survey. <br /> • Retained the services of Advanced Technology Laboratories (ATL), a Caltrans-approved and <br /> California-certified analytical laboratory located in Signal Hill, California, to perform the <br /> chemical analysis of soil samples. <br /> • Caltrans provided notification to the adjacent private property owner in advance of the field <br /> activities. <br /> Field Activities <br /> Geophysical Survey <br /> On September 6, 2013, AGS performed a geophysical and utility survey at the Site. AGS identified <br /> shallow metal at the UST location at Excavation 1 in addition to a buried metal anomaly and various <br /> utilities. The findings of the AGS survey are depicted on Figure 2. A copy of AGS's geophysical <br /> survey report is attached. <br /> Soil Sampling <br /> We performed two soil borings at the Site on November 18, 2013 using Geocon's direct-push rig. <br /> Borings B 1 and B2 were performed on either side of the tank to a maximum depth of 16 feet. Each <br /> boring was initially advanced using a hand-auger to a depth of 5 feet to confirm clearance of buried <br /> utilities and the tank. The approximate boring locations are depicted on Figure 2 and shown on <br /> Photo No. 5. <br /> Continuous core soil samples were collected in 4-foot-long cellulose thermoplastic (acetate) liners <br /> driven by the direct-push rig at depths between 5 and 16 feet. The acetate liners were cut to obtain soil <br /> samples at 5-, 10- and 15-foot depth intervals. The liners were capped with Teflon® sheets and <br /> polyethylene end caps. Each sample container was then labeled and placed in a cooler pending delivery <br /> to ATL for chemical analyses under chain-of-custody(COC)protocol. <br /> Linden UST Site investigation,TO No.4 Caltrans Contract 06A1895,EA No. 10-OY3600 <br /> Geocon Project No.89800-01-04 -2- December 11,2013 <br />
