CONTRACTOR'S QUALIFICATIONS <br /> The Contractor responsible for destroying the well shall possess a valid California C-57 Water Well <br /> Contractors License.Subcontractors shall be lawfully licensed for the work they are contracted to <br /> compete. <br /> COORDINATION/COOPERATION <br /> The Contractor shall be responsible for contacting and coordinating with all utility companies with <br /> regards to the location of existing underground facilities in the construction area.The Contractor <br /> shall call Underground Service Alert at (800) 642-2444, at least 2 working days before <br /> commencement of underground work for location of underground facilities. <br /> PRESERVATION OF PROPERTY <br /> The Contractor shall use such means as are necessary to confine all work to the project area. <br /> Utility facilities damaged,temporarily disconnected, abandoned or relocated as a result of work <br /> shall be repaired/reconnected or properly abandoned as directed by the governing utility at the <br /> Contractor's expense. <br /> PERMITS,BONDS,LICENSES AND INSURANCE <br /> The Contractor shall procure all permits, bonds, licenses and insurance, pay all charges and fees, <br /> and give all notices necessary and incidental to the prosecution of the work. <br /> The Contractor shall prepare, pay applicable fees,and submit the Well Destruction Application(s) <br /> as required by SJCDEH. <br /> PUBLIC CONVENIENCE AND SAFETY <br /> The Contractor shall be responsible for assuring that the work site is safe and secure and that <br /> temporary coverings of well casing,open holes,and/or excavations are provided whenever there <br /> is an interruption in work.All excavations shall be secured and protected from the public,animals, <br /> and debris when not under direct control of the Contractor. <br /> REMOVAL WELL Of ABOVE GROUND FEATURES <br /> The Contractor shall remove the well pedestal and concrete apron of the monitoring well. <br /> Contractor shall remove all materials generated as part of the well destruction. <br /> SEALING MATERIAL <br /> The sealing material shall consist of: <br /> Neat Cement <br /> Materials: a) Potable Water <br /> b)Type I or II Portland Cement <br /> Mixture: 4.5 to 6 gallons of water to 94-pounds of Portland cement <br /> LSCS <br />
