CLOSURE REQUEST REPORT Section No 2 0 <br /> Former American Forest Products Company 07/31/01 <br /> 2801 West Lane Page 2-3 <br /> Stockton California <br /> q0 <br /> A five-year groundwater monitoring plan was submitted to the PHS-Em in December 1995, which was <br /> approved by the PHS-EHD in March 1996 An annual report of groundwater monitoring was issued in <br /> October 1996, which reported the results of monitoring performed in September 1996 (Dames & Moore, <br /> 1996) In September 1997, Dames & Moore issued the Annual Report, 1997 Groundwater Monitoring, <br /> which reported the results of groundwater monitoring performed in January and July of 1997 (Dames & <br /> Moore, 1997) <br /> In August I998, PHS-EHD indicated that the five-year monitoring program needed to be reevaluated as <br /> an approach to closure, because rising groundwater levels might be approaching impacted soil As a <br /> result, PHS-EHD required further investigation and documentation, including the installation of new <br /> monitoring wells and quarterly groundwater monitoring for one year In September 1998, Dames & <br /> Moore (1998a) submitted a soil and groundwater investigation work plan to PHS-EHD, and issued an <br /> annual report of groundwater monitoring, which documented monitoring activities conducted in January <br /> and July of 1998 (Dames & Moore, 1998b) Two new wells (DMW-6 and DMW-7) were installed in <br /> December 1998, and quarterly monitoring resumed During the time since the two wells were installed in <br /> December 1998, ten quarterly groundwater monitoring rounds have been conducted <br /> On August 10, 2000, Ms Carol Oz of the PHS-EM instructed URS to initiate site closure procedures <br /> As part of the closure process, The PHS-EHD requested that quarterly monitoring including the collection <br /> and analysis of groundwater samples for oxygenates and 1,2-dichloroethane (1,2-DCA) These <br /> parameters were included as part of the third quarter 2000 monitoring event, conducted on August 25, <br /> 2000 After review of the August 2000 analytical results, the PHS-EHD requested that an additional set <br /> • of groundwater samples be analyzed for oxygenates and 1,2-DCA These parameters were included with <br /> the fourth quarter monitoring program, conducted on November 15, 2000 An additional monitoring <br /> event was conducted on March 1, 2001 <br /> K 1840191afp\afp_c1osure_rpt doc <br />
