Pursuant to Section 2652 (d) of the Underground Storage Tank Regulations <br /> (California Code of Regulations, Title 23, Chapter 16) a quarterly monitoring <br /> report (QMR) shall be submitted to PHS/EHD and CVRWQCB until the investigation <br /> and cleanup of the site is deemed adequate. The report is to show the: <br /> -status of investigation and cleanup activities including the <br /> results of all investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the <br /> completion of the investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance <br /> and any contaminated soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, <br /> conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if known, and <br /> changes in the above measurements over the last quarter. <br /> The QMR shall be submitted no later than June 30, 1993 . <br /> If you have any questions, please contact Diane Hinson, Supervising RENS, of my <br /> staff at (209) 468-3452. <br /> Jogi Khanna, M.D.ZREHS, <br /> M.P.Hr <br /> Health <br /> Officer <br /> �Laurie .
