IL <br /> PETER D'AMICO <br /> HOROLD FODY <br /> PAGE 2 <br /> Within 45 days from the reopening of the station, Thrifty Oil Company will initiate the <br /> work described in the workplan and the workplan addendum. <br /> Within 30 days of the completion of the field work, groundwater sampling and vapor <br /> extraction testing shall be initiated at the site. <br /> Within 60 days of the completion of the groundwater sampling and vapor extraction <br /> testing, a report of findings shall be submitted to PHS/EHD. <br /> The concrete debris observed in the former tank pit area was discussed. The disposal <br /> receipt for the concrete debris shall be submitted to PHS/EHO immediately. <br /> Additionally, under the direction of PHSIEHD, the former tank pit area shall be <br /> trenched to either find and remove the debris or to indicate the debris is not present in <br /> sufficient volume to be a hinderance to future assessment/remediation efforts. The <br /> trenching shall be initiated immediately after the stockpiled soil is removed from the <br /> site. <br /> The existing piping shall be removed and the soil beneath the piping sampled <br /> immediately upon the setting and backfilling of the new underground tanks. The <br /> placement of the new piping shall be such that it will not be a hinderance to future <br /> assessment/remediation efforts. <br /> All soil generated from the site is to be disposed of appropriately and disposal receipts <br /> shall be submitted to PHSIEHD within 30 days from the disposal date. <br /> "As built" pians showing the accurate placement of the tanks and piping shall be <br /> submitted to PHSIEHD within 30 days from the completion of the construction. <br /> To prevent additional misunderstandings, all future communications for scheduling and <br /> inspections for the new tank installation shall be made by either Peter D'Amico of <br /> Thrifty Oil Company or Harold Fody of Premier Petroleum and shall be directed only to <br /> either Doug Wilson or Diane Hinson of this office. <br /> Failure to abide by the above written terms, as agreed upon at the administrative <br /> hearing, or to schedule required inspections, or to provide at least 48 hours advance <br /> notice of inspections, will result in the termination of site work and may lead to future <br /> enforcement action. <br /> If you have any questions or wish to discuss this letter in more detail, contact Diane Hinson, <br /> Supervising REHS, at (209) 468-3452 or Doug Wilson, Supervising RENS, at (209) 468-3446. <br /> rnest M.
