the HazMat Group into two teams: the HazMat Team and the Decon Team. The Group's <br />responsibilities are divided into three general areas, which include. <br />• Hazards identification. <br />• Hazards containment/control and rescue. <br />• Decontamination of personnel and equipment. <br />4.6.8 Site 300 Medical Service <br />The Site 300 medical facility is located in Building 889. It is typically staffed during normal <br />operating hours, Monday through Thursday. The medical facility number is extension 3-5250. <br />For emergency assistance, workers call 911 (from an onsite LLNL phone) or (925) 447-6880 <br />(cell phone or offsite phone). <br />The Livermore site and Site 300 Fire Departments are also emergency medical service (EMS) <br />first•responders. In the event the medical facility is closed, emergency aid can be provided by the <br />Fire Dept. at B890. In addition, the LLNL Health Services Department at the Livermore Site has <br />a medical clinic facility and a decontamination unit at Building 663. An individual contaminated <br />with hazardous substances may be brought to the medical facility to undergo decontamination <br />and emergency medical treatment. Call Health Services at 4224459 for services and operational <br />hours. <br />4.7 Facility Manager, Representative, or Designated Alternate <br />The Facility Manager or Representative manages small incidents at their respective Facilities. <br />The Facility Manager coordinates all incident response measures and has the authority to commit <br />resources needed to mitigate small incidents as described in this Contingency Plan. <br />During large incidents, the Facility Manager, Representative, or alternate provides information <br />regarding wastes stored at the Facility to the IC when requested. The information provided by <br />waste records includes identification of wastes involved, quantities, and Facility status. The <br />Facility Manager or Representative shall also develop and implement, in conjunction with the <br />EFA, actions needed to meet environmental requirements related to the incident. <br />The Facility Manager or Representative has the following specific responsibilities during Facility <br />incidents: <br />• Evaluate the immediate scope of the incident. <br />• Initiate evacuation of personnel, if necessary. <br />• Notify the Fire Department. <br />• Take appropriate action to safeguard Facility personnel. <br />• Ensure that the ES&H Team Environmental Analyst, Assurance Office, and the Health <br />and Safety Technician have been notified. <br />• As temporary IC, direct area personnel in accordance with the Facility Safety Plans <br />(FSP) and Contingency Plan until the Fire Department and the official IC arrive. <br />• Ensure that all normal waste handling operations cease in areas <br />within and bordering <br />the incident until cleanup and/or control procedures are completed. <br />LLNL Site 300 Contingency Plan 17 January 2019 <br />
