All written reports will include the following: <br />• Name, address, and telephone number of the owner or operator (LLNL or LLNS). <br />• Name, address, and telephone number of the RHWM Facility. <br />• Date, time, and type of incident (e.g., release, fire, or explosion). <br />• Name and quantity of materials) involved. <br />• Extent of injuries, if any. <br />• Assessment of actual or potential hazards to human health or the environment, when <br />this is applicable. <br />• Estimated quantity and disposition of recovered material that resulted from the incident. <br />All written reports will be reviewed and forwarded to the appropriate regulatory agency by the <br />EFA Manager. <br />12.2.3 Verbal and Written Notification to Agencies for Releases from a Permitted <br />Hazardous Waste Management Facility <br />Whenever a release occurs from a Facility requiring the implementation of the Contingency Plan, <br />verbal notification to DTSC is required. In addition, a written report must be submitted within IS <br />days. <br />All verbal and written reports include the following: <br />• Name an <br />d telephone number of the reporter. <br />• Name and address of the Facility. <br />• Date, time, and type of incident (e.g., release or fire). <br />• Name and quantity of materials) involved, to the extent known. <br />• Extent of injuries, if any. <br />• Possible hazards to human health or the environment outside the LLNL Site 300 facility <br />boundaries. <br />• Actions taken and status of Facility. <br />DTSC must also be informed that no waste that may be incompatible with the released material <br />is transferred, treated, stored, or disposed of until cleanup procedures are completed and that all <br />emergency equipment listed in the Contingency Plan is cleaned and fit for its intended use before <br />operations are resumed. <br />12.2.4 Department of Energy Reporting <br />All verbal and written notifications to DDE for hazardous and toxic releases are completed in <br />accordance with LLNL Implementation Procedure for Reporting Occurrences to DOE (UCRL. <br />MA433867). <br />LLNL Site 300 Contingency Plan 33 ,January 2019 <br />
