The following is an itemized list of underground storage tank violations that have not <br /> been addressed for GRANT LINE SHELL* as of May 12, 2020. <br /> Open violations from May 30, 2019 inspection <br /> Violation#209 -Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days. <br /> The designated operator failed to document the following: <br /> On the 10/26/18, 11/21/18, 12/19/18 designated operator(DO) inspection report under <br /> -Section II I <br /> The DO failed to add a description to all the components marked N/A or N from section VII through XI. <br /> On the 1/17/19, designated operator(DO) inspection report under <br /> - Section III <br /> The DO failed to add a description to all the components marked N/A or N from section VII through XI. <br /> -Section VI <br /> The OWNER/OPERATOR failed to provide at least one of the following: a name, signature, and date within 48 <br /> hours of receiving the DO report under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section. <br /> On the 4/10/19 and 5/8/19 designated operator(DO) inspection report under <br /> -Section III <br /> The DO failed to add a description to all the components marked N/A or N from section VII through XI. <br /> -Section V <br /> The owner/operator failed to provide explanation of follow-up actions that will be taken for the statement provided by <br /> the DO in section III. <br /> -Section VI <br /> The OWNER/OPERATOR failed to provide a name, signature, and date within 48 hours of receiving the DO report <br /> under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section. <br /> Furthermore the DO did not conduct an inspection with 30 days of the previous inspection. The DO inspected the <br /> facility on 1/17/19 and on 2/18/19. The DO report was conducted 32 days after the 1/17/19 inspection and not within <br /> 30 days as stated by regulation. <br /> During the designated operator inspection, the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing inspections at least once every 30 days at this facility and are including all of the required information on <br /> the reports. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#319 -No spill bucket,failed to contain 5 gallons, no means to keep it empty, or failed to be <br /> tested. <br /> The 87 Product, 89 product, and diesel product tanks UST is not equipped with spill containers that contain a <br /> minimum of 5 gallon capacity. All USTs shall be equipped with a spill container that has a minimum capacity of five <br /> gallons and a drain valve that allows drainage of the collected spill into the primary container or provide a means to <br /> keep the container empty. The spill containers listed above were tested for the 60 minute hydrostatic test with <br /> passing results, however the spill containers do not meet the five gallon requirement. Immediately contact a properly <br /> licensed, trained, and certified contractor to install an approved spill container under permit, if required, and <br /> inspection of the EHD. <br /> Page 4 of 5 <br />
