Item # Remarks 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1 , 2018), OBSERVATION: The Facility Employee Training Certificate dated 612/2020, did not include Mark A Pope date of assuming responsibility as a facility employee. REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018, or within 30 days of hire if prior to October 1 , 2018, The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 205 CCR 2712(i) , 2634(e) Failure to submit a current UST Response Plan and have it available onsite. OBSERVATION: An approved copy of the response plan was not available for inspection. REGULATION GUIDANCE: A copy of this plan shall be accessible on site at all times. CORRECTIVE ACTION: Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: - The 07/5/2019 DO report did not include the owner/operator name, owner/operator signature and date signed. REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation.
