Madison Site <br />#809 <br />Location: Dock <br />Issue: Unable to demonstrate the amount of time used fluorescent bulbs and electronic wastes <br />have been stored on site <br />Action: The electronic waste was removed via EWASTE4GOOD. Please see item #7 about our <br />actions with the fluorescent bulbs. <br />9. 123 W. Sonora Site <br />#122 & 705 <br />Issue: Manifest numbers 002477348JJK and 004911272JJK were found without a signed off <br />copy from the destination facility <br />Action: Attached is manifest 004911272JJK with the designated facility. Since manifest <br />002477348JJK was dated back in 2008, Evergreen Oil, Inc. has to go back to their paper <br />archives. We will have it within the next few weeks, but as per the requirement of the <br />EHD, we have sent in an Exception Report to the DTSC Report Repository. (see <br />attached) <br />11. 123 W. Sonora Site <br />#125 <br />Issue: Copies of used oil records for 2010 and 2011 not found on site. <br />Action: Evergreen Environmental Services has been contacted and is going through their <br />archives. They will send them to us when they find it (within the next two weeks) and <br />we will, in turn, send them to you. <br />12. 123 W. Sonora Site <br />#701 <br />Location: Hazardous Waste Storage Area <br />Issue: A 55 -gallon drum had an accumulation start date of 23 February 2011 <br />Action: As the 55 -gallon drum had used oil in it, we had it picked up by Evergreen <br />Environmental Services. See the Work Order/Service Agreement No. 636204 attached. <br />13. 123 W. Sonora Site <br />#128 <br />Issue: Bills of lading for the recycling used oil filters for 2010 and 2011 were not found on site. <br />Action: Please see attached Certificate of Disposal from CleanHarbors. This pickup consisted of <br />all the oil filters, among other hazardous waste items, from the past 2 years.
