Emergency <br />Response Form Attached. <br />109 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 <br />days. <br />Generator copies of manifest number 005328995SKS (2/29/16) was not sent to DTSC <br />within 30 days of shipment of hazardous waste. The generator manifest copy shall be <br />submitted to DTSC for every shipment on a manifest when California is either the generator <br />state or the destination or consignment state. The generator manifest copy shall be <br />mailed to: <br />DTSC Generator Manifests <br />P.O. Facilities who generate less than 1000 kg of <br />hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time <br />may store waste on site up to 180 days. Since it cannot be determined how long the <br />hazardous waste has been on site, immediately contact a licensed hazardous waste hauler <br />to dispose of this waste under manifest. The drum was shipped off-site by a licensed waste hauler on <br />9/20/17.
