Unable to determine the length of storage of at least 14 universal waste fluorescent <br />lamps on the second floor and one lamp in the alley, two universal waste CRTs in the alley, and broken <br />pieces of a universal waste fluorescent lamp (see item #301 above). Neither the lamps nor CRTs were <br />not stored in containers. Universal wastes, including aerosol cans, lamps, alkaline batteries, mercury <br />containing switches, CRT, electronic devices, etc. shall be stored in a closed container that prevents <br />breakage and release to the environment, labeled to identify the type of Universal Waste and the initial <br />date of accumulation, managed within one year of storage, and retained a copy of the disposal receipt <br />for at least 3 years on site. Immediately dispose of the above universal waste lamps and CRTs, and <br />submit a copy of the disposal receipt to EHD by June 1, 2012. A pamphlet for the Small Hazardous <br />Waste Disposal Facility has been supplied to the generator during this inspection. <br />812. Copies of Universal Waste disposal records were not found on site. A copy of the disposal receipts <br />of Universal Waste, including but not limited to lamps, alkaline batteries, mercury containing switches, <br />CRT, and electronic devices, shall be retained on site available for inspection. Immediately locate a copy <br />of all Universal Waste disposal records for the last three years, maintain them on site, and submit copies <br />to the EHD by June 1, 2012. <br />NOTES (The following are not violations) <br />The facility generated 420 pounds of hazardous waste in 2011 based on one used oil disposal record. <br />CUPA class schedule and hazardous waste labeling factsheet were supplied during this inspection. <br />404. One used oil container not closed. All hazardous waste containers shall be closed at all times <br />except when adding or removing waste. Discrepancy was corrected on site. <br />703. An emergency coordinator and modified contingency plan information was lacking. There must be <br />at least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm.
