The incorrectly filled out consolidated manifests include the following invoice numbers: 90371 from 3-31-16, 88689 from 12-28-15, 086994 from 9-22-15, 084766 from 5-29-15, 083087 from 2-24-15, 81370 from 11-19-14, 79724 from 8-27-14, 75853 from 1-30-14. The Maximum Oil Service consolidated manifest with invoice#35542 lacked the state ID#of the hazardous waste generator. The generator and waste sections of the uniform manifest shall be completed and signed and dated by the generator. The transporter shall also sign and date the manifest.
