The UW handler shall label or mark UW to identify the type of UW. Lamps(i.e. each lamp), or a container in which the lamps are contained, shall be labeled or marked clearly with the following phrase: "Universal Waste-Lamp(s)". Correctly immediately by properly labeling all UW lamps/containers of UW lamps as "Universal Waste-Lamp(s)". Minor -notice to comply. #84 The UW lamps in the blue poly drums did not have documentation as to how long they have been a UW. UW handlers must be able to demonstrate the length of time that the UW has been accumulated from the date it became a waste and properly dispose of the UW within 1 year of the accumulation start date. Correct immediately by ensuring that your UW has documentation that indicates the accumulation start date. Properly dispose of the UW lamps per CCR Title 22 HW regulations. Submit a copy of the disposal record for the UW to this office by 10-28-10. Minor -notice to comply. #85 The disposal records for the UW lamps from the last three years were not available for review. The UW waste handler shall keep a record of each shipment of UW sent form the UW handler's facility to another facility. The record may take the form of a log, invoice, manifest, bill of lading or other shipping document. The record for each shipment of UW sent shall include the following information: (1) The name and address of the UW handler or destination facility to which the UW was sent; (2) The quantity of each type of UW sent; (3) The date of departure of the shipment of UW; A UW handler shall retain each record for at least three year from the date of departure of the corresponding shipment of UW. Correct immediately by maintaining these records available for review. Submit a copy of these documents from the past three years to this office by 10-28-10. UW fact sheet & CUPA class list provided. Complete & submit return to compliance form, correction statements, & documents to EHD by 10-28-10. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
