The following are Repeat / Minor <br />Violations - Notice to Comply: #7,20,27,30,50,60,63,82,83,84,85 <br />7. Following wastes require HW determination: <br />Unknown substance in one unlabeled 55 -gallon metal drum, one unlabeled 40 -gallon poly drum, <br />and one unlabeled 30 -gallon poly garbage can, <br />o Oily liquid (used oil and oil-based coolant) in one unlabeled and open 40 -gallon poly drum outside <br />the building by the shipping area. <br />Generator shall make HW determination of the above waste and managing the waste accordingly, <br />Immediately make a HW determination of the above wastes and manage each accordingly, or dispose <br />of the waste as_ HW in accordance to California Code of Regulations (CCR) Title 22, If the waste is <br />determined to be a non -HW, you are required to submit evidence (i.e, third party laboratory testing <br />results) supporting such a determination to FH®. Submit a statement and supporting documentation <br />explaining how this waste was managed by March 14, 2011. <br />20 & 50. Observed five 5 -gallon buckets, three 1 -gallon jars, and multiple cans of used oil beneath the <br />shelves in the building. All HW containers shall be marked with the following information, <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6, accumulation start date <br />* Immediately label these containers and ensure that all containers are marked with all the required <br />information. Since these containers have been on site longer than 180 days, immediately contact a <br />licensed HW hauler to dispose of this waste under manifest and submit a copy of the manifest to the <br />FH® by March 14, 2011. <br />27. Three of the 5 -gallon buckets mentioned above in Item #20 & 50 were not closed, <br />-- All HW containers shall be closed at all times except when adding or removing waste. <br />* Immediately close these containers and ensure all HW containers are closed when not adding or <br />removing waste, <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EH Inspe r: <br />Received By: <br />Title: <br />T <br />AD 23-02-J003 0 Rev W11 0/10 CONTINUATION FORM <br />
