Failed to conduct daily inspections of HW tank. At time of inspection: <br />Facility failed to perform daily inspections and maintain written daily logs of the hazardous waste tank(s) The owner/operator <br />shall inspect, and document, where present, at least once each operating day: <br />1. Overfill/spill control equipment <br />2. Aboveground portions of the tank system to detect corrosion or releases of water <br />3. Data gathered from monitoring equipment <br />4. Outside areas of the tank system including secondary containment to detect erosion or signs of releases of hazardous <br />waste <br />5. The level of waste in the tank (for uncovered tanks) <br />6. Cathodic protection if present <br />Immediately begin daily inspections, including documentation. Within 30 days submit to this office a copy of the inspection <br />log. <br />43. Failure to retain waste analysis for 3 years. At time of inspection facility was managing their brake lathe shavings as a <br />non -hazardous waste. According to Philip Thomas, Manager Environmental Affairs, a hazardous waste analysis, per Title 22 <br />requirements, had been run on this waste stream. Within 30 days provide this office with a copy of the analysis which <br />demonstrates this waste stream is non -hazardous. <br />45 & 47. Contingency plan incomplete/No emergency coordinator. At time of inspection: <br />The contingency plan on site was incomplete. The contingency plan must include: <br />1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non -sudden release of <br />hazardous waste to air, soil or surface water at the facility. <br />2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and State and <br />local emergency response teams to coordinate emergency services. <br />3. List name, address, and phone numbers (office/home/cell) of all persons qualified to act as emergency coordinator. <br />Where more than one person is listed, one shall be named as the primary emergency coordinator and all others in order in <br />which they will assume responsibilities. <br />4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities. <br />5. Evacuation routes for facility personnel where there is a possibility of evacuation. <br />6. Current telephone number for State Office of Emergency Services. <br />If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency plan, the <br />owner or operator need only amend that plan to incorporate hazardous waste management provisions that are sufficient to <br />comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill the requirements of this <br />chapter.
