NIFIRST-CO14PORATION
68 Jonspin Rd, Wilmington, Massachusetts 01887
ACCIDENTAL DISCHARGE / SLUG CONTROL PLAN
ISSUED: 03-03-04
ISSUED BY: Stephen Aquilino
REVISION: 1.a
PURPOSE
l*eOID
The purpose of this Accidental Discharge / Slug Control Plan is to establish an approved method of safely
preventing, containing, and countering process wastewater and chemical spills that may occur on the
premises of Unifirst Corporation in Stockton, California. The Slug Plan will also establish procedures
for reporting significant discharges to appropriate government agencies. These procedures will be
accomplished by delegating responsibilities to employees and/or private contractors. These
responsibilities include containing and reporting the discharge, cleanup, and notifying the appropriate
government agencies. Such a plan is necessary in order to comply with government regulations and to
minimize hazards to human health or the environment
This ADSC Plan is intended to provide guidance for responding to accidental discharges which involve
liquid and solid materials and which may occur on the premises of UniFirst Corporation. The ADSC Plan
is located in the Plant Manager's office and is readily available for guidance. Response procedures in
this ADSC Plan offer guidance for responding to a single uncontrolled discharge that has the potential of
releasing hazardous substances into the sanitary sewer. Emergency coordinators are instructed to call
the local POTW for guidance in the event that mixtures of materials are discharged into the sewer.
UniFirst Corporation intends to comply with Stockton Municipal Code Section 7-089.1 and EPA
regulations concerning SLUG CONTROL by following the guidelines set forth in 40CFR 403.8(f)(2)(v).
1.0 Management Approval
This ADSC Plan will be implemented as herein described. The Emergency Coordinator has the
authority to commit Unifirst Corporation resources as may be required to protect public health and
safety and/or the environment.
(Signature) (Date)
Scott Beaton Plant Manager
(Name printed) (Title)
2.0 Facility Information
Name: UniFirst Corporation
Location: #370
Number of Employees 45
Hours of operation 6:00 am — 5:00pm
Discharge periods- 6:00 am-3:00pm
Mailing Address: 819 Hunter Street, Stockton, California 95202
Telephone:( 209) 941-8364
Emergency Coordinator: Scott Beaton
Home Telephone: (209) 368-8576
Home address: 2133 Bluejay Way, Lodi, CA 95240
Secondary Emergency Coordinator: Rod Thomas
