Reports shall be submitted to the City of Stockton Environmental Control <br />section no later than the I& day of the following month for which monitoring was conducted. <br />The volume of discharge shall be recorded daily. Reports shall include total daily flow and total monthly flow, in gallons, <br />discharged to the sewer. <br />pH shall be reported by exception on a monthly basis. By exception shall mean reporting the minutes per day and value <br />thereof for any pH reading that is out of compliance. All instances of noncompliance for pH must be accounted for and <br />reported in the monthly report. Technician performing the calibration will record the date and time on the pH recording chart <br />and initial the entry. <br />Permittee shall insure that all handling, preservation, and laboratory analysis of collected samples is performed in <br />accordance with 40 CFR Part 136 and amendments thereto, unless otherwise specified in the monitoring conditions of this <br />permit. <br />24 hour flow proportional composite sampling shall be performed on a continuous basis. <br />The monthly reports will also indicate the time, date and place, of sampling, persons responsible for collection and handling <br />of the sample including all chain of custody reports, methods of analysis, date of analysis, sample analysis results including <br />quality control results for the analysis and shall certify that such sampling and analysis is representative of the expected <br />pollutants discharged to the sewer. Should the permittee monitor forany pollutants more frequently than required by this <br />permit, including, but not limited to, spills, site remediation work, stormwater, internal treatment system audits, or treatment <br />operational testing the results from those monitoring events shall also be included in the monhly report. <br />Should monitoring by the permittee indicate a violation with any part or condition of this permit, Permittee must notify <br />the Environmental Control Section within24 hours of becoming aware of the violation. Any instances of failure to <br />report by the permittee may result in the assessment of maximum fines allowed by the Stockton Municipal Code. <br />Permittee shall devise a schedule of rotating days for sampling purposes, subject to approval by the City of Stockton <br />Environmental Control Section. An example of rotating days for once per week would be Monday of the first week, Tuesday <br />of the second week, Wednesday of the third week, etc. <br />The monthly report and any compliance reports required by this permit shall contain the following certification <br />statement signed by an authorized representative: "I certify under penalty of law that this document and all attachments <br />were prepared under my direction or supervision in accordance with a system designed to assure that qualified <br />personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who <br />manage the system, or those persons directly responsible for gathering the information, the information submitted is, to <br />the best of my knowledge and belief, true, accurtte, and complete. I am aware that there are significant penalties for <br />submitting false information, including the possibility of fine and imprisonment for knowing violations." <br />The permittee shall perform the City of Stockton, Department of Municipal Utiliies, Quality Assurance Manual for <br />Self -Monitored Industries in conjunction with permittee Sel€Monitoring Sample Program. <br />
