UniFirst Corporation <br />Self -Monitoring Sample Program <br />Page 4 of 4 <br />Permittee shall have all flow monitoring equipment calibrated and certified by a licensed technician at a minimum frequency <br />of once per six months. All documentation relating to monitoring equipment calibration and certification shall be included in <br />the self-monitoring report for which the work was performed. <br />The permittee shall not completely or partially bypass any waste stream around any pretreatment equipment, or transfer and <br />mix any waste into the effluent holding tanks, nor shall the permittee add potable or process water to them, or in any way <br />attempt to dilute as partial or complete substitute for adequate treatment to achieve compliance with limitations contained <br />within this permit. <br />The permittee shall not tamper, obstruct, disconnect, or bypass any monitoring equipment used for determining compliance <br />with any condition contained within this permit. The permittee must report to the Environmental Control Section within 24 <br />hours of becoming aware of any interruption or malfunction to monitoring equipment required by this permit. <br />The permittee shall take all reasonable steps to minimize or correct any adverse impact to the Stockton Wastewater Control <br />Facility or the environment resulting from noncompliance with this permit, including such accelerated or additional <br />monitoring as necessary to determine the nature of the noncomplying discharge. <br />The permittee shall furnish to the Municipal Utilities Department, within a reasonable time, not to exceed 30 days unless a <br />written time extension is approved by the Municipal Utilities Department, any relevant information which the department <br />may request to determine whether cause exists for modifying, revoking, reissuing, or terminating this permit, or to determine <br />compliance with this permit. The permittee shall also furnish to the Municipal Utilities Department, upon request, copies of <br />any records required to be kept by this permit. <br />Misrepresentation or failure to disclose all relevant facts in any required reports shall be considered a violation of this <br />permit. <br />
