6.2.3 The Emergency Coordinator will further assess possible hazards to human health or the environment that <br />may result from the release. This assessment must consider both direct and indirect effects of the <br />emergency (i.e., the effects of any toxic, irritating or asphyxiating gases that are generated or the effects <br />of any hazardous surface water run-off). <br />6.2.4 If it is determined that the emergency could threaten human health or the environment within the facility, <br />then plant evacuation should take place immediately. <br />6.2.5 If the assessment indicates that a release has occurred which does not threaten the environment, public or <br />employees and which exceeds our in-house capabilities to safely and appropriately respond to the spill, <br />then evacuation should still take place and the local authorities contacted. <br />7 After the Emergency <br />Within 5 days of the incident, the Emergency Coordinator (or designee) must submit a written report of the <br />incident to the California State EPA. The report must include the following information: <br />• Name, address and telephone number of the person filing the report. <br />• Name, address and telephone number of the facility at which the incident occurred. <br />• Date, time and location of the incident. <br />• A brief description of the circumstances surrounding the incident. <br />• A description and estimated quantity, by weight or volume, of materials or wastes involved. <br />• An assessment of any contamination of land, water or air that has occurred due to the spill incident. <br />• Estimated quantity and disposition of recovered materials or wastes that resulted from the incident. <br />• A description of what actions the management of the facility intends to take to prevent a similar <br />occurrence in the future. <br />The California State EPA report is to be sent to: PO Box 806 <br />Sacramento CA, 95812 <br />The City of Stockton further requires a written follow up report to be filed by the generator within five calendar <br />days of the incident, with the Pretreatment Section. The report shall contain the following: <br />A description of the discharge, the cause of the upset and the upsets impact upon the user's compliance <br />status. <br />The period of noncompliance, including the exact dates and times of noncompliance, and if the <br />noncompliance continues, the time by which the noncompliance is expected to be corrected. <br />Any and all steps taken, or to be taken, to reduce, eliminate and prevent recurrence of the <br />noncompliance. <br />The City's written report is to be sent to: <br />8 Chemical Substances at UniFirst — Stockton, CA <br />Michelle Price, Environmental Control Officer <br />2500 Navy Drive <br />Stockton CA 95206 <br />There are eight chemicals used in the laundering process. These chemicals are all stored in a designated areas <br />located in the washroom. Chemicals are manually, and automatically, drawn from storage as needed. The <br />following chemicals and their properties are described in the attached Manufacturers Safety Data Sheets <br />(MSDS). Aside from the wash aisle chemicals, the only potentially hazardous materials are located in the waste <br />water room and the boiler room. <br />
