Fremont Street <br />Stockton, CA 95205 <br />I. CASE SUMMARY <br />Fremont Shell is a gasoline dispensing facility that consists of a 15,000-gallon <br />underground storage tank (UST) holding 87-octane gasoline, a 10,000-gallon UST <br />holding 91-octane gasoline, and four dispensers. Both tanks and all sumps are double <br />walled and monitored with hydrostatic fluid. The piping is all double walled and <br />monitored with vacuum. <br />On July 27, 2011, Mr. Jeffrey Wong, Senior Registered Environmental Health Specialist <br />(REHS), San Joaquin County Environmental Health Department (EHD), performed a <br />routine UST inspection (Attachment 1) and witnessed the annual monitoring system <br />certification, leak detector, and spill container testing. Approximately one cup of liquid <br />was found in each of the fill sumps. Brine levels were low in the 87-octane and 91- <br />octane piping sumps, the 91-octane fill sump, and the vent sump annular spaces, and <br />their corresponding sensors were in alarm and could not be cleared. During a review of <br />the facility's paperwork, Mr. Wong found that the facility's designated operator, Ms. Lori <br />Freshour, did not have a valid International Code Council (ICC) certification to perform <br />the work of a designated operator and three alarms were not noted in the designated <br />operator monthly inspection reports. Ms. Freshour's certification expired in November <br />2009, and the monthly inspection reports by Ms. Freshour lacked complete <br />documentation for alarms dating back to 2008. The EHD UST inspection reports from <br />2009 and 2010 are attached for reference (Attachments 2 and 3). <br />On July 29, 2011, a monitoring system certification test report was submitted, but it <br />incorrectly indicated that all the sensors passed testing (Attachment 4). Mr. Wong called <br />the testing company, Henderson Maintenance Company, and asked Mr. Wayne <br />Henderson to submit a revised report.
