T The following is an itemized list of hazardous waste violations that have not been addressed for Quick Stop Mart as of June 09, 2015. Open violations from April 21,2015 inspection Violation#208-Designated operator monthly inspection reports not maintained on site for at least 12 months. The designated operator monthly inspection reports for the past twelve months were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. Violation#209-Designated operator employee training not performed or log not kept. The designated operator employee training was not performed. No training logs were found on site. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. Violation#210-Unstaffed facility failed to have or implement a training program approved by the EHD. The designated operator employee training was not performed. For facilities that are not routinely staffed, the designated operator shall implement an employee training program approved by the EHD. Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. Violation#301 -UST system is not operated to prevent unauthorized release, including spills and overfills. The facility is not currently in operation. The monitoring panel is currently displaying sensor out alarms for all three sensors at the facility (1-1, L2, L3 -the sumps sensors for both STP sumps and the tank annular sensor). According to Mr. Dhillon, the wiring was cut several weeks ago when someone broke into the facility. The monitoring system is down and the tanks are not currently being monitored. The underground storage tank system shall be operated to prevent unauthorized releases. Immediately correct by doing either of the following: - Have a properly licensed, trained, and certified contractor repair or replace the monitoring system in order to restore continuous tank monitoring, with functional components(LG 113-x listed, if applicable), and obtain a permit within one business day from the EHD. -Obtain a permit from the EHD to temporarily close this UST system and perform the closure under inspection by the EHD. Violation#302-UST system incapable of detecting leak from primary into secondary containment(pre- Jul 2003). The annular sensor is not functional and, therefore, the tank is not monitored. The facility is not currently in operation. The monitoring panel is currently displaying sensor out alarms for all three sensors at the facility(1-1, L2, L3 -the sumps sensors for both STP sumps and the tank annular sensor). The interstitial space shall be monitored with a monitoring system connected to an audible and visual alarm system. Immediately correct by doing either of the following: -Have a properly licensed, trained, and certified contractor repair or replace the monitoring system in order to restore continuous tank monitoring, with functional components (LG 113-x listed, if applicable), and obtain a permit within one business day from the EHD- -Obtain a permit from the EHD to temporarily close this UST system and perform the closure under inspection by the EHD.
