The following is.an itemized list of hazardous waste violations that have not been addressed <br /> for Quick Stop Mart as of June 09, 2015. <br /> Open violations from April 21, 2015 inspection <br /> Violation#306 -Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> The 87 STP sumps sensor, 91 STP sump sensor, and annular sensor were found to be in alarm since April 9, <br /> 2015 and actions have not been taken to address the alarm. All monitoring equipment shall be maintained to be <br /> able to detect a leak at the earliest possible opportunity. Immediately contact a properly licensed, trained, and <br /> certified contractor to address the alarm under permit and inspection of the EHD. <br /> Violation#307 -Monitoring system of the interstitial space of a double walled system not maintained. <br /> The annular sensor is not functional and, therefore, the tank is not monitored. The facility is not currently in <br /> operation. The monitoring panel is currently displaying sensor out alarms for all three sensors at the facility(L1, <br /> L2, L3 -the sumps sensors for both STP sumps and the tank annular sensor). According to Mr. Dhillon, the <br /> wiring was cut several weeks ago when someone broke into the facility. The monitoring system is down and the <br /> tanks are not currently being monitored. The interstitial space shall be monitored with a monitoring system <br /> connected to an audible and visual alarm system. Immediately correct by doing either of the following: <br /> - Have a properly licensed, trained, and certified contractor repair or replace the monitoring system in order to <br /> restore continuous tank monitoring, with functional components (LG 113-x listed, if applicable), and obtain a <br /> permit within one business day from the EHD. <br /> -Obtain a permit from the EHD to temporarily close this UST system and perform the closure under inspection by <br /> the EHD. <br /> Violation#308-Failed to maintain a continuous monitoring system that activates an audible and visual <br /> alarm. <br /> The STP sump sensors are not functional and, therefore, the tank system is not monitored. The facility is not <br /> currently in operation. The monitoring panel is currently displaying sensor out alarms for all three sensors at the <br /> facility(1-1, L2, L3-the sumps sensors for both STP sumps and the tank annular sensor). The monitoring panel is currently displaying sensor out alarms for all <br /> three sensors at the facility (1-1, L2, L3-the sumps sensors for both STP sumps and the tank annular sensor). <br /> According to Mr. Dhillon, the wiring was cut several weeks ago when someone broke into the facility. The <br /> monitoring system is down and the tanks are not currently being monitored. No record was available of daily <br /> inspections of the monitoring panel for alarms; according to Mr. Dhillon, he is checking on the system daily, but <br /> forgot his log at home. He stated that he does not leave the log at the facility since it has been vandalized a lot <br /> lately. No person shall close an UST system without complying with the requirements of HSC 25298. Billa Dhilon <br /> , the tank owner's son and the current site supervisor, states the UST system will be reused. He stated that the <br /> property has been sold with escrow due to close next Wednesday, April 29, 2015. Immediately correct by doing <br /> either of the following: <br /> - Have a properly licensed, trained, and certified contractor repair or replace the monitoring system in order to <br /> restore continuous tank monitoring, with functional components(LG 113-x listed, if applicable), and obtain a <br /> permit within one business day from the EHD. <br /> -Obtain a permit from the EHD to temporarily close this UST system and perform the closure under inspection by <br /> the EHD. <br /> Page 4 of 7 <br />
