0 <br /> The following is an itemized list of hazardous waste violations that have not been addressed <br /> for Quick Stop Mart as of June 09, 2015. <br /> Open violations from April 1, 2014 inspection <br /> Violation#104-Current financial responsibility documents not on file with the Environmental Health <br /> Department(EHD) <br /> Financial responsibility documents for 2013 have not been submitted to the EHD. The most recent financial <br /> responsibility records on file are dated March 28, 2012. Current financial responsibility documents are required to <br /> be submitted annually. Complete and submit a copy of the financial responsibility by May 1, 2013. <br /> Violation#108 -Secondary containment: (a)test not performed; (e)test report not submitted within 30 <br /> days; (f)test notification <br /> not provided to the EHD at least 48 hours prior to conducting the test <br /> Secondary containment testing was last performed on November 19, 2009 and was due by November 2012. <br /> Testing has not been completed and is five months past due. Secondary containment testing is required once <br /> every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. <br /> Violation#205-Designated operator monthly inspection reports not maintained on site for at least 12 <br /> months <br /> The June 2012 designated operator monthly inspection report was not found on site. Designated operator <br /> monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that <br /> copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. <br /> Submit copies to the EHD by May 1, 2013. <br /> Page 7 of 7 <br />
