a <br /> The following is an itemized list of hazardous waste violations that have not been addressed <br /> for Quick Stop Mart as of June 09, 2015. <br /> Open violations from April 7, 2014 inspection <br /> Violation #107 -Plot plan/site map not submitted or fails to completely show where monitoring is <br /> performed <br /> A plot plan/ monitoring site map is not submitted in CERS. All monitoring programs shall include the location(s), <br /> as identified on a plot plan, where the monitoring will be performed. Immediately log into CERS, update the <br /> required information, and submit for review by the EHD. <br /> Violation#201 -Maintenance&monitoring records not maintained 3 years, cathodic protection records <br /> for 6%years,written performance claims pertaining to release detection systems for 5 years, or records <br /> of repairs, linings, and upgrades for the life of the tank. Monitoring records include: (1)date and time of <br /> all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of <br /> any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all <br /> readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br /> results of inventory readings and reconciliations <br /> Monitoring and maintenance records were not found on site for the cold start inspection conducted on September <br /> 4, 2013 or the alarm noted in the alarm history for the 87 STP sensor on February 28, 2014. These records shall <br /> be kept on site for at least 3 years. Immediately locate and maintain the maintenance and monitoring records on <br /> site and submit copies to the EHD by May 7, 2014. <br /> Violation#202-Monitoring plan, response plan, and/or operating permit and all conditions not onsite <br /> A copy of the 2014 UST operating permit was not found on site. A copy of the UST operating permit and all <br /> attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it <br /> on site. REPEAT VIOLATION <br /> Violation#204-Designated operator monthly inspection: (1)alarms not documented; (2-4)spill <br /> containers, UDCs, and sumps <br /> (if necessary) not inspected; (5) required test dates not checked or noted; (6)employee training not verified <br /> The designated operator failed to check and note the correct test dates for the SB989 (triennial secondary <br /> containment)testing on the designated operator monthly inspection reports. The test date is documented on the <br /> reports as November 1, 2012; according to EHD records, this testing took place on April 10, 2013. During the <br /> monthly inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> Violation#205-Designated operator monthly inspection reports not maintained on site for at least 12 <br /> months <br /> The April, August, September, October, November, December 2013 and January, February, and March 2014 <br /> designated operator monthly inspection reports were not found on site. Alarm history printouts for these months <br /> were found in the designated operator binder on site. Designated operator monthly inspection reports for the <br /> previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of <br /> designated operator monthly inspection reports are maintained on site. Submit copies to the EHD by May 7, <br /> 2014. REPEAT VIOLATION <br /> Violation#303 -UST monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity <br /> The 87 wrap-around annular sensor was tested and passed at the time of inspection, however the technician <br /> could not get the sensor back in the tank. The 87 annular sensor is currently not located at the bottom of the tank <br /> to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak <br /> at the earliest possible opportunity. Immediately contact a properly licensed, trained, and certified contractor to <br /> relocate this sensor to the lowest point of the 87 tank. <br /> Page 6 of 7 <br />
