Financial responsibility documents that have been updated were last signed on 6/12/2017 and are no longer current. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/,and upload the required documents. This is a repeat violation, Class II. 105 CCR 2632(d)(1), 2634(d)Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the 89 product tank has been converted into the 91 product tank and the monitoring plan has not been updated on CERS to demonstrate those changes. Different leak detectors were observed for both tanks. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes,and submit for review by the EHD. This is a repeat violation, Class II. 111 CCR 2638(a)Failed to test monitoring and leak detection equipment annually. The 91 product leak detector testing were last performed on 6/13/2016 and was due by June 2017. Testing has not been completed and is 24 months past due. These tests are required once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. This is a repeat violation, Class II. 113 CCR 2638(d)Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 6/26/17 and a test report was submitted on 12/04/17 , 4 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 121 CCR 2712(f)Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 6/26/17 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 7/26/17. This information was received on 1/19/18, 5 months late, resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a Class II violation.
