Environmental Health Department <br /> S A N.3J O A Q U I N AMENDED <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> TIWANA GAS&FOOD 1 1210 E HAMMER LN, STOCKTON June 29,2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 201 CCR 2712(b)Failure to maintain monitoring, maintenance, and follow up action records. <br /> Maintenance and monitoring records for the last three years were not found on site. On the 4/3/2018 designated <br /> operator report the designated operator stated that the L5 went into alarm on 3/29/2018 and asked the site to <br /> dispatch this alarm however no record of the follow up was found on site. These records shall be maintained on site <br /> for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring <br /> equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis <br /> performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water <br /> elevations,or other test results; (6)results of inventory readings and reconciliations. Immediately locate and <br /> maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a Class II violation. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br /> and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br /> This is a Class II violation. <br /> 207 HSC 25294 Failed to record unauthorized release from primary containment. <br /> When the inspector and service technicians arrived on site approximately 30 gallons of fuel was observed in the 91 <br /> STP sump was not reported to the EHD within 24 hours and a full report has not been submitted to the EHD within <br /> five working days.The monitoring sensor(L5)has been in active alarm since 3/29/2018.The service technician <br /> tested the functionality of the sensor and verified that it was working properly.The site owner was aware of the <br /> alarm but stated that they had not been using the tank since the L5 went into alarm, however it was unclear whether <br /> anyone had addressed the alarm since 3/29/2018. No maintenance records of this alarm being addressed were <br /> found on site.Any release from the primary containment that does not escape the secondary containment and was <br /> not cleaned up within eight hours of detection, or any release that escapes the secondary containment shall be <br /> reported to the EHD within 24 hours and a full report shall be submitted within five working days. The report shall <br /> include at least the following information: <br /> 1. Owner's or operator's name and telephone number <br /> 2. List of types, quantities,and concentrations of hazardous substances released <br /> 3. Approximate date of release <br /> 4. Date release was discovered <br /> 5. Date release was stopped <br /> 6. Description of actions taken to control and stop the release <br /> 7. Description of corrective and remedial actions <br /> 8. Methods of clean up implemented to date, and proposed clean up actions with a time schedule <br /> 9. Method and location of disposal of waste generated from release and clean up. Attach copies of any <br /> hazardous waste manifests. <br /> 10. Description of proposed methods of repair or replacement of primary or secondary containment <br /> 11. Description of additional actions taken to prevent future releases. <br /> Immediately prepare a report with the required information and submit a copy to the EHD.
