When the inspector and service technicians arrived on site approximately 30 gallons of fuel was observed in the 91 STP sump was not reported to the EHD within 24 hours and a full report has not been submitted to the EHD within five working days.The monitoring sensor(1-5)has been in active alarm since 3/29/2018.The service technician tested the functionality of the sensor and verified that it was working properly.The site owner was aware of the alarm but stated that they had not been using the tank since the L5 went into alarm, however it was unclear whether anyone had addressed the alarm since 3/29/2018. No maintenance records of this alarm being addressed were found on site. Any release from the primary containment that does not escape the secondary containment and was cleaned up within eight hours of being detected shall be recorded with the following information: 1. Operator's name and phone number 2. List of types,quantities, and concentrations of hazardous substances released 3. Description of actions taken to control and clean the release 4. Method and location of disposal of the released substances 5. Description of actions taken to repair and prevent future releases 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment Immediately record this release with the information required, and maintain it on site with the facility's monitoring records. Submit a copy of the recordable release statement to the EHD. This is a Class II violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to visually inspect the 91 STP sump for leaks,water, or debris during the 4/3/2018, 5/2/2018, and 6/1/2018 monthly inspection. During the monthly inspection,the designated operator shall visually inspect all spill containers and UDC sumps,and any tank top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators performing monthly inspections at this facility are inspecting all of the required areas and noting it on the on the reports. The designated operator failed to check and note the correct test dates for the secondary containment testing on the June 2017-June 2018 designated operator monthly inspection report.The designated Operator stated that the test was conducted on 10/3/2017 and 7/8/14 however EHD records show the routine secondary containment testing to have occurred on 7/31/17. During the monthly inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a Class 11 violation.
