All components installed must be certified by an independent testing organization <br />and bear the appropriate mark as required by the independent testing <br />organization. <br />5. If concrete is broken near the UST, pipeline and tank integrity testing must be <br />performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br />guidelines. Results must be received by the EHD within seven days of returning the <br />facility to operating condition. <br />6. A "Certification of Installation/Modification" form shall be submitted by the owner or <br />owners' agent before this upgrade will be considered complete. <br />7. This permit expires 180 days from the approval date. <br />8. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
