UST Facility Operating Permit is not current in CERS. The Board of Equalization (BOE) number, 44040786, is closed and not valid. Contact the Board of Equalization at 1-800-400-7115 to get a valid number. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a repeat violation, Class Il. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. Beneath "Tank Monitoring is Performed Using the Following Method(s)", enter "Dry" for "Secondary Containment System" for both the 91 and 87 tanks. Beneath "Under Dispenser Containment (UDC) Monitoring", for both the 91 and 87 tanks, "UDC Monitoring Stops Flow of Product at Dispenser' is incorrectly marked "Yes," and should be marked as "No." The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class Il. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The L1 liquid sensor should be associated with the 87 -unleaded subersible turbine pump (STP) sump and the L3 sensor should be associated with the 87 -unleaded annular space-, the L2 sensor should be associated with the 91 -supreme STP sump and the L4 sensor should be associated with the 91 -supreme annular space. Also, the approximate location of the underground storage tank piping is missing from the site plan. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II. 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 2/27/2017 and a test report was submitted on 4/24/2017 , over 3 weeks late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner.
