11 The following is an itemized list of underground storage tank violations that have not been addressed for SHOP N GO 3 as of August 21, 2017. Open violations from May 10, 2016 inspection Violation #103 - Current financial responsibility documents not submitted. Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. Violation #111- Failed to test monitoring and leak detection equipment annually. Annual monitoring system certification and leak detector testing were last performed on February 10, 2015 and was completed on May 10, 2016 , three months past due. These tests are required once every 12 months. Take all necessary precautions to ensure testing is performed in a timely manner. Violation #112 - Failed to test spill containers annually. Annual spill container testing was last performed on February 10, 2015 and was completed on May 10, 2016 , three months past due. This test is required once every 12 months. Take all necessary precautions to ensure testing is performed in a timely manner. Violation #113 - Failed to submit monitoring system certification test report within 30 days. Annual monitoring system certification, leak detector testing, and spill container testing was performed on February 10, 2015 and a test report was submitted on March 27, 2015 , 1/2 month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The 2013 and 2015 monitoring system certification test results. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. Violation #209 - Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the monitoring system certification test and the spill containment testing on the February 2015 through April 2016 designated operator monthly inspection report. The incorrect date listed 2-17-15. The correct date on file is 2-10-15. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done.
