SUMMARY OF VIOLATIONS CLASS I, CLASS 11, or MINOR -Notice to Comply) On site to witness the annual monitoring system certification testing and conduct a routine inspection. Class II Violations: 109, 204, 206, 302, and 304. 109. Annual monitoring system certification, leak detector testing, line integrity test, and spill container testing was performed on 11-10-10 and a test report was submitted to the EHD on 11-15-11. A copy of the test report must be submitted within 30 days of the tests. Ensure that a copy of the test reports are submitted to the EHD within 30 days of the tests. 204. The designated operator failed to check and note the test date for the line test on the 11-10-10 and 12-15-10 designated operator monthly inspection reports. The designated operator listed the monitoring certification, spill container, and leak detector testing as 11-12-10 on the 12-15-10 designated operator monthly inspection report. It should have been listed as 11-10-10. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 204. The designated operator failed to attach the alarm history report to the 1-24-11, 3-9-11, and 10-6-11 designated operator monthly inspection reports. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 206. Before today's training, the designated operator employee training for Marcos Ortega III last occurred on 11-25-09 and for Kelli Kishida on 7-15-10. Marcos Ortega's training is considered two years late. Kelli Kishida's training is considered four months late. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
