Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan. <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan. <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms. <br />Ensure that employees have been trained by the designated operator every twelve months and maintain <br />the list on site. <br />302 & 304. The diesel annular sensor failed to activate an audible and visual alarm when tested and, <br />therefore, the tank is not monitored. The interstitial space shall be monitored with a monitoring system <br />connected to an audible and visual alarm system. Immediately contact a properly licensed, trained, and <br />certified contractor to replace the failed component with a functional component (LG 113-x listed, if <br />applicable) under permit and inspection of the EHD within one business day. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 12-15-11. <br />Note: Additional copy of the monitoring and response plan were provided during the inspection.
