Beckman Road, Lodi, CA 95241-0357, Service Request #: SR0032755 <br />Your permit application to remove the product lines, monitoring system, vent lines and vapor recovery lines, <br />dispensers and pump islands, and to install double wall fiberglass piping, piping sumps, monitoring system, <br />dispensers, under dispenser containments, and fill/vapor system at the above referenced location has been <br />approved with the following conditions: <br />Removal Conditions: <br />1. Contact the local fire department for their permit requirements. <br />2. Samples must be taken and analyzed as stated on pages 4 and 5 of the permit application. (Note: <br />GeoAnalytical Labortary will perform the soil sampling and the analytical tests.) <br />3. EHD must witness the soil sampling. Contact EHD to arrange this inspection appointment. <br />4. The nearby storm drains shall be protected from any possible contamination. <br />Installation Conditions: <br />1. Contact the local fire department and San Joaquin Valley Air Pollution Control District for applicable <br />permits. <br />2. Any changes in information or scope of work must be submitted to EHD prior to commencing work. <br />3. Submit certificate of training issued by the tank and piping manufacturers prior to commencing work. <br />4. All piping and connections shall bear an UL label. <br />5. All piping must slope to a containment sump with an audible and visual alarm. <br />6. All equipment in contact with backfill must have corrosion protection. <br />7. Overfill prevention must satisfy CCR 2635(b)(2). <br />8. If automatic pump shut down is provided it must satisfy 2636(g) of California Code of Regulations <br />(CCR). <br />9. Contact EHD for the following inspections: <br />a. Sampling. <br />b. Primary piping. <br />c. Secondary piping. <br />d. Piping slope. <br />e. 24-hour lake test on dispenser pans and sumps. <br />f. Final inspection: <br />i. A final inspection must be performed prior to returning this station to operation. <br />ii. A third party certified tank integrity test must be performed and results received prior <br />to scheduling the final inspection. <br />Page I of 2 <br />
