r <br />0 <br />iii. A third party certified piping line tightness test must be performed and results received <br />prior to scheduling the final inspection. <br />iv. Submit a "Certificate of Compliance for Underground Storage Tank Form C" prior to <br />scheduling the final inspection. <br />v. Submit an As -Built plan to EHD at the time of the final inspection. Note: A final <br />inspection will not be performed unless the as-builts are submitted. <br />10. Contact Steven C. Shih, R.E.H.S. at (209) 468-9850 to arrange all inspections. Allow 48-hour notice <br />for all inspections. <br />11. Permit expires within 90 days. Note: If an extension to the expiration date is required a request must <br />be received in writing to this office prior to the expiration date. <br />12. Secondary containment systems installed on or after January 1, 2001 shall be tested upon installation, <br />6 months after installation, and every 36 months thereafter. Secondary containment systems installed <br />prior to January 1, 2001 shall be tested by January 1, 2003 and every 36 months thereafter. <br />Thank you, <br />Steven C.
