The L4 diesel annular and L5 87 annular sensors alarmed on February 25, 2013. The designated operator did not document these alarms on the monthly designated operator report nor were they documented on the facility alarm log. These records shall be kept on site for at least 3 years. Immediately locate and maintain the facility alarm log and submit copies to the EHD by February 28, 2014. 204. The designated operator failed to document all the alarms from the attached alarm history on the March 11, 2013, designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: the L4 diesel annular and L5 87 annular sensors which alarmed on February 25, 2013. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 204. The designated operator failed to check and note the correct test dates for the monitoring system certification, leak detector, and spill container tests on the November 5, 2013 designated operator monthly inspection report. The test date was listed as 1-1-13, but should have been listed as 1-28-13. The designated operator failed to check and note the correct test dates for the monitoring system certification and spill container tests on the June 2013 through October 2013 designated operator monthly inspection reports. The test date was listed as 1-23-13, but should have been listed as 1-28-13. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operator performing monthly inspections at this facility are including all of the required information on the reports.
