The monitoring plan on site is not current. The monitoring plan is missing the Red Jacket FX1 V leak <br />detector and the Veeder Root 205 sensor. The monitoring and response plans must be current and <br />approved by the EHD. Complete and submit a copy of the monitoring plan for approval by the EHD by <br />February 28, 2013. An approved copy will be returned for your records. <br />203. Diesel fuel was leaking from the diesel filter inside dispenser #3/4 on January 28, 2013, was not <br />recorded. Any release from the primary containment that does not escape the secondary containment <br />and was cleaned up within eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the EHD by February 28, <br />2013. <br />305. The sensor in the 91 turbine sump failed to stop the flow of product at the dispenser when tested. <br />All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br />product at the dispenser when it detects a leak. During the inspection, the technician replaced the 91 <br />turbine relay. Positive shutdown of the 91 turbine was verified during the retest of the 91 sensor, sensor <br />out/fail safe and power out functionality tests. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD 23-02 Rev 04/19/12 1 / Page 3 UST INSPECTION REPORT <br />
