A new designated operator, Benjamin Webb, conducted the April 8, 2014 designated operator monthly inspection, and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a minor violation. 123 Permit to repair UST system not obtained. The monitoring panel was cold started and the 89 annular sensor was replaced on May 19, 2014 and a permitted inspection was not conducted with the EHD. The permit already existing for a cold start conducted on May 9, 2014 was amended to include the additional cold start and the replacement of the 89 annular sensor, but no inspections were scheduled or conducted for the additional work. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Ensure that all future repairs are done under a permit from the EHD in compliance with all permit conditions. This is a Class II violation. 206 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the triennial secondary containment testing on the June 2014, July 2014, and August 2014 designated operator monthly inspection report. The test date listed is April 29, 2014, which was the date of six month retest for the repaired STP sump. The date of triennial secondary containment testing for this facility is July 25, 2013. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class Il.
